CAMPER REGISTRATION

All information provided in this registration form is true, complete and accurate.

I consent to the collection, use and disclosure of personal information provided in this registration for the purposes of camp administration, safety and communication.

I authorize camp staff to administer basic first aid and, if necessary, seek emergency medical treatment for the camper. I understand that all medications must be provided in original containers with proper labeling.

I understand that participation in camp activities involves inherent risks, including but not limited to physical injury and property loss. I voluntarily assume all such risks on behalf of the camper and release the camp, its staff, and affiliates from any liability for injury, accident, or loss, except where prohibited by law.

I acknowledge that the camp reserves the right to dismiss any participant for misconduct or failure to comply with camp rules, without refund.

I grant permission for the camp to photograph or record the camper during activities and to use such media for marketing, promotional and communication purposes, unless I provide written notice opting out.

I have read and understand the Refund Policy noted above.

I understand that while reasonable care will be taken during transportation, the camp is not responsible for incidents beyond its control during transit.

I agree that by submitting this form electronically—whether by clicking “Submit,” checking consent boxes, or typing my name—I am providing my consent and acknowledgment, which shall have the same legal effect as a handwritten signature.
